The resulting process of chronic disease is an inability to perform activities independently and freely it takes a caregiver who provides care settings. Optimal caregiver role requires an approach FCC Model appropriate to enhance these capabilities. One model that can be applied to the caregivers of stroke patients is family centered care. This study aims to develop a FCCmodel for caregivers who care for stroke patients at home and can be used as a guide. This study was action research, which consists of stages reconnaissance, planning, acting, observing and reflecting. Methods of data collection is done through focus group discussions, questionnaires and participant observation.Participants who contributed in this study were 31 caregivers of stroke patients. Data analysis using quantitative and qualitative methods. The qualitative data obtained through focus group discussions, and observations, while quantitative data were collected through questionnaires FCC and stroke patients swallow status assessments and observations, analyzed using descriptive statistical tests. This research has resulted in the FCC model which is used in the treatment of stroke patients with dysphagia disorders in SimalingkarPublic Health Center (PHC) Medan, and a positive impact on increasing the skills, ability to swallow training and assessment capabilities in treating stroke patients. Another impact of the results of the development is a caregiver has been able to apply when treating stroke patients with dysphagia disorders.Researcher expects the PHC Management to implement Family Centered Care Model and conduct supervision for performance evaluation.
